Transition Management Services (TMS)

Our Transition Management Services are designed to support individuals with mental health conditions and substance use disorders as they move toward more independent living. Through a team-based, person-centered approach, we help individuals successfully transition from institutional or unstable settings into safe, stable housing in the community. Our dedicated team provides housing support, life skills coaching, peer support, and care coordination to empower each person in reaching their goals and maintaining long-term stability.

Outpatient Therapy

Our Outpatient Therapy Services are for children, adolescents, and adults experiencing mental health, substance use, or co-occurring challenges. Our licensed therapists offer individual, family, and group therapy in a safe and supportive environment. Through evidence-based practices, we help individuals build coping skills, improve emotional well-being, and achieve personal goals on their journey toward recovery and stability.

Group Therapy

Our Group Therapy is a supportive and therapeutic space for individuals to connect with others facing similar challenges. Led by licensed clinicians, our groups focus on building coping skills, enhancing communication, and promoting personal growth. Group therapy addresses a variety of mental health and substance use needs, encouraging shared learning, mutual support, and a sense of community in the healing process.

Diagnostic Assessment

We provide Diagnostic Assessments to determine the presence of mental health, substance use, intellectual, and developmental disabilities (IDD) conditions. Completed by licensed clinicians, this evaluation involves clinical interviews and screening tools to establish a diagnosis and assess the need for services. The results guide the development of individualized treatment plans that support each person’s unique path to stability and wellness.

Comprehensive Clinical Assessment

We provide Comprehensive Clinical Assessments (CCAs) for children, adolescents, and adults to evaluate mental health, substance use, and intellectual and developmental disabilities (IDD) needs. Conducted by licensed clinicians, the assessment includes a thorough review of an individual’s medical, psychological, and social history to determine diagnoses and recommend appropriate services. Our goal is to develop personalized, strength-based treatment plans that promote recovery, independence, and overall well-being.

1915i Respite

Our 1915(i) Respite Services is a home and community-based service (HCBS). We provide essential short-term relief for primary caregivers of individuals with mental health needs, IDD, or TBI. Respite care gives families and caregivers a break while ensuring their loved one is cared for in a safe, supportive environment. We believe in delivering personalized care with respect, empathy, and integrity, helping families restore balance and peace of mind. Services are tailored to meet the unique needs of each individual, helping to reduce caregiver burnout and enhance the overall well-being of the family.

This service helps caregivers caring for people with

  • Serious emotional disturbance (SED), Substance Use Disorder (SUD) ages 3-20 

  • Intellectual and developmental disabilities (I/DD), traumatic brain injury (TBI); ages 3 or older
